Job Description:

LSF is looking for a talented Host Homes Case Manager who would
like to make an impact in the lives of others.
Purpose & Impact:
The Youth Host Home Case Manager helps youth and young adults
identify and achieve goals related to maintaining safe, stable
housing, employment, education, and increasing life skills. The
Youth Host Home Case Manager has responsibility for: providing case
management to youth in the program (pre, during and post host home
residency), supporting positive relationships between hosts and
youth and supporting the overall development and implementation of
the program. In conjunction with the Host Home Coordinator, the
Youth Host Home Case Manager will develop professional
relationships and partnerships with other community service
providers and provide community engagement or program outreach as
required.
Essential Functions:
* Provide ongoing supportive case management, housing match, crisis
assistance, and conflict resolution and stabilization services to
youth enrolled in host home program.
* Establish Individual Development Plans in partnership with youth
that address recovery, emotional and psychological issues,
vocational and educational development, employment, financial
management, and prioritization of needs.
* Assist youth in obtaining and maintaining public benefits.
Provide referrals for and solve problems related to county, state
and federal benefits programs.
* Identify, apply for and establish appointments for available
services such as cash aid, food programs, medical clinics and
in-home support. Accompany youth to critical appointments and or
meetings when deeper support is needed.
* Establish a transition plan with youth to ensure they move into
preferably long-term stable housing at the end of host home
residency.
* Support intake, assessment and evaluation for youth engagement in
program.
* Document youth progress records in a timely manner, maintain and
monitor their files and/or database. Document all outcomes. Create
reports based on information compiled. Provides direct care and
supervision of youth. Conduct safety and wellness checks and home
visits regularly.
* Assist with building and refining program policies and procedures
to continually finetune the program.
* Assist with identifying, screening and training volunteer
community members who will volunteer to host clients.
* Maintain strong relationships with collaborative partners and
hosts. Attend multi-agency and partner meetings. Assist youth in
negotiating conflicts with partner agencies.
* Cultivate a culture of learning: attend regular, ongoing
trainings related to topics connected to Host Homes, including
supporting youth experiencing housing instability, positive youth
development, harm reduction and trauma-informed care.
* Comply with all policies and procedures.
* Perform other related duties as assigned
Other Functions:
* All duties are performed in accordance with the following
standards:
* Experience working in a collaborative setting and demonstrated
ability to form relationships working within a multi-ethnic,
multi-culture youth staff and public environment.
* Ability to work within a harm reduction model and experience
working with trauma informed programs as well as strong experience
providing services addressing homelessness, violence, substance
use, mental health, and criminal justice systems.
* Understanding of the needs of vulnerable minority communities,
strategies to meet their needs, and ability to work with a broad
range of constituents, program participants, and stakeholders
representing the full diversity of the community. A champion for
building pathways for people not served by mainstream systems.
* Strong communication, conflict resolution and de-escalation
skills, ability to maintain confidentiality.
* Ability to work independently, collaboratively with supervisor or
as part of a team in a fast-paced environment. Experience working
collaboratively with multiple agencies desired
* Computer/PC literacy, including Internet proficiency, Microsoft
Office software and networked systems.
Physical Requirements:
* Must have a high level of energy, be adaptable to irregular
hours, and be able to travel as needed.
Education:
* Minimum Bachelor's degree in social work or related field
Experience:
* 3 years of experience in case management, counseling and working
with runaway, homeless and at-risk youth.
Skills:
* Must possess excellent written and verbal communication
skills.
* Must be familiar with and able to use Microsoft Office programs
Word and Excel.
* Ability to form relationships and effectively work with
multi-ethnic, multi-culture youth, staff, and the general
public.
* Ability to handle crisis situations and adapt to every changing
environment.
* Demonstrated skills in assessment, crisis intervention,
behavioral management techniques, individual and family counseling
practices and group process.
* Ability to work as a team player and work flexible hours as
needed in a 24/7 residential program.
* Demonstrates skills in organization, time management and
attention to detail.
* Recognizes the need to request assistance, consultation or
feedback on difficult cases or challenges within the agency.
Other:
* Must demonstrate sensitivity to our service population's cultural
and socioeconomic characteristics and needs.
