Job Description:

LSF is looking for a talented Shelter Case Manager who would like to make an impact in the lives of others. Purpose & Impact: The shelter case manager is responsible for the case management, counseling, and HHS activities for runaway and homeless youth in all aspects of the basic center program. The shelter case manager works closely with the Homeless Trust partners and collaborates with these agencies in service linkage, community meetings, HHS activities, referrals, and service delivery. The shelter case manager coordinates prevention and intervention services for RHY and works closely with those agencies specializing in services for sexual exploitation. The shelter case manager works in conjunction with other residential counselors to ensure smooth interagency program coordination. The shelter case manager assists the Residential Director in ensuring that all requirements of the HHS grant are met. Essential Functions:

  • Implements all aspects of case management services eg completion of the intake process, completion of all required assessments, development of a service plan, documentation of case notes and progress summaries
  • Responsible for developing strategies for finding a permanency option for the homeless youth
  • Works with youth following their 21 day exit and acceptance into CINS/FINS services
  • Conducts individual counseling for the youth and thoroughly and clearly documents progress in case notes of the youth's file
  • Responsible for ensuring the youth are involved in all activities of the basic center program and are receiving services as outlined in their service plan
    • Participates and prepares reports for the weekly multidisciplinary team meetings.
    • Leads case discussion that focuses on each youth's needs, behaviors, academic achievement, and conducts, provided services, aftercare plan and progress toward discharge
    • Completes all required semi-annual progress reports per grant requirements
    • Maintains accurate, clear, and thorough data on RHYMIS and uploads data timely in adherence to requirements
    • Assists the Residential Services Manager in the development and implementation of the HHS activities for RHY program
      • Establishes and maintains on-going relationships with community providers who provide services for runaway and homeless youth.
      • Attends all community meetings related to the Homeless Trust and Miami Homes for All initiatives
      • Maintains quality assurance standards as it relates to the BCP grant requirements, licensing requirements and agency standards
      • Completes training requirements of RHY program through RHYTAC and through other agency funders
      • Maintains training log and submits certificates timely
        • Works in collaboration with other residential counselors to ensure appropriate intra-agency functioning
        • Attends and participates in all agency meetings as required Other Functions: All duties are performed in accordance with the following standards:

          • Safety: Employee makes a reasonable effort to adhere to established safety procedures and practices in the work area.
            • Training: Attends and successfully completes all mandated training courses with RHY, DCF, DJJ.
            • Confidentiality: Adheres to all confidentiality rules. Physical Requirements:
              • Must have a high level of energy, be adaptable to irregular hours, and be able to travel as needed. Education:
                • Minimum Bachelor's degree in social work or related field Experience:
                  • 3 years of experience in case management, counseling and working with runaway, homeless and at -risk youth. Skills:
                    • Must possess excellent written and verbal communication skills.
                      • Must be familiar with and able to use Microsoft Office programs Word and Excel.
                      • Ability to form relationships and effectively work with multi-ethnic, multi-culture youth, staff, and the general public.
                      • Ability to handle crisis situations and adapt to every changing environment.
                      • Demonstrated skills in assessment, crisis intervention, behavioral management techniques, individual and family counseling practices and group process.
                      • Ability to work as a team player and work flexible hours as needed in a 24/7 residential program.
                        • Demonstrates skills in organization, time management and attention to detail.
                        • Recognizes the need to request assistance, consultation or feedback on difficult cases or challenges within the agency. Other:
