Description Case Manager RN will assume responsibility for caseloads consisting of patients in specialty areas, conduct initial assessment to obtain thorough physical, social and environmental data, establishes the plan of care, provides direct care and adjusts nursing care processes as needed. Advises and consults with family and other personnel as appropriate. Must have excellent communication skills, good organizational skills, strong interpersonal skills and the ability to work with a variety of clients with patience and respect. Requirements

• Current Compact or FL State license
• AHA BLS
• Three years of previous experience Shift
