At Landmark, our interdisciplinary teams collaboratively manage our
complex patient panels. These teams are led by Physicians, Nurse
Practitioners, and Physician Assistants, with supporting care
provided by RN Nurse Care Managers, Social Workers, Pharmacists,
Behavioral Health, and other employed team members. Join Landmark

The Field Based Healthcare Ambassador is a pivotal member of the
Landmark Interdisciplinary Team. This role actively engages with
patients both in the home and by telephone. The incumbent will use
compassion and empathy to earn patients' and caregivers' trust,
persistence to support and advocate for them, and excellent
communication skills to build impactful relationships.
This position is full-time Monday - Friday. Employees are required
to have flexibility to work any of our 8-hour shift schedules
during our normal business hours of 8:00am - 5:00pm EST. It may be
necessary, given the business need, to work occasional
overtime.
Primary Responsibilities:
- Conduct home visits and/or phone or video calls to patients to
collect clinical and psychosocial information, and carry out
designated tasks, including but not limited to:
- Resource referral and connection
- Quality measure documentation, support, and follow up
- Telemedicine facilitation and set up
- Collect information regarding patient conditions using
standardized checklists and templates.
- Partner with Landmark interdisciplinary team (providers,
nurses, social workers, etc.) to drive improvement in patient
outcomes and satisfaction
- Actively participate in weekly interdisciplinary team
meetings

Required Qualifications:
- High School Diploma / GED (or higher)
- 3+ years of experience in any of the followings job categories:
Home Health Aide; Certified Nursing Assistant; Certified Medical
Assistant; Medication Aide; Health Coach; Health Navigator or
Healthcare Technician
- Ability to conduct independent patient home visits, OR have
prior experience making home visits to patients
- Experience with MS Office products including Word (draft, save
and share documents), Excel (create, save, share and use basic
formulas), Power Point (create presentations, save and share) and
Outlook (draft emails, manage calendars)
- Live or reside within a commutable distance in Miami FL (Miami
Dade county)
- Ability to travel 75%
- Full COVID-19 vaccination is an essential job function of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

Qualifications:
- Experience with geriatric or primary care experience, or
experience in institutional settings of care such as long-term
care/skilled nursing facilities
- Social Worker experience Soft Skills:
- Exceptional interpersonal skills. Superb written and oral
communication skills, and comfort level with technology. Physical
and Work Environment:
- Regular requirement to sit; use hands to touch, handle or
feel.
- Regular requirement to talk and listen.
- Occasional requirement to stand; walk and reach with hands and
arms
- Occasional requirement to lift and/or move up to 10
pounds.
- Vision abilities include close vision, peripheral vision, depth
perception and ability to adjust focus
- Ability to travel frequently by car To protect the health and
