About the Client: Physicians who work for this organization have a non-compete in their contract; however, it is limited to capitated practices only. A capitated medical practice receives a fixed, predetermined payment for each patient assigned to them over a specific period, typically on a per-member, per-month basis, regardless of how many services the patient uses. The care model is built around value-based, preventive medicine with a focus on proactive care, chronic disease management, and strong physician-patient relationships. The organization partners with Medicare Advantage health plans to deliver coordinated care designed to improve outcomes and reduce hospitalizations. Physicians work within a team-based model that includes nurses, care coordinators, behavioral health specialists, and social workers, allowing providers to focus on prevention and long-term health management rather than high-volume episodic care.
